Want to save time? As a registered patient of Health Service, you can now schedule appointments with your Health Service physician online. Through this portal system, you will also be able to view some medical test results that have been released by your physician on your own computer.

To take advantage of this convenience, you need to complete a Patient Acknowledgement and Agreement form the next time you come to the clinic.  You can download and print form, complete it and bring it in the next time you come to the clinic. Once you’ve done that, you will receive an e-mail message with your password and information about how to establish online access.

Once registered, users can book appointments and access their medical records through the my.healthandwellness.utoronto.ca link. This link will take the registered user (accessed by password) to an online program where they can view their lab results/book appointments.
